Resources for Human Development (RHD) is hiring a Registered Nurse (RN) Staff Nurse to join the Delaware Intensive Case Management team. The role emphasizes community-based care for individuals with mental illness and co-occurring conditions, including medication management and comprehensive assessments.

The RN will work with an interdisciplinary team, provide education, coordinate care, and support participants in achieving independence.

Qualifications

• Current RN license in Delaware.
• Graduate of an accredited School of Nursing.
• CPR certification and successful background check.
• Ability to travel throughout the community to provide services.

Responsabilités

• Nursing Assessment & Clinical Care: conduct psychiatric, clinical, and physical health assessments and provide nursing care.
• Medication Management: administer medications and educate participants about treatments.
• Community-Based Support & Education: provide nursing services in homes and community settings and educate families.
• Care Coordination: coordinate care with physicians, pharmacies, and other providers.
• Crisis & Emergency Support: coordinate crisis intervention and maintain clinical judgment in emergencies.
• Documentation, Safety & Compliance: complete clinical documentation and ensure HIPAA compliance.
