Job summary

Hammer Residences is seeking a Staff Development Nurse - RN to promote health and wellbeing for adults with intellectual and developmental disabilities in group homes and apartments. You will train staff, monitor health care plans, and ensure compliance with 245D and related regulations.

The role involves occasional evenings/weekends and a rotating emergency call line, with a primary focus on person-centered care.

Qualifications

• Licensed as an LPN or RN in good standing with the State of Minnesota.
• Valid driver’s license and driving record meeting Hammer Residences requirements.
• MN DHS background clearance and negative TB test result.
• Excellent written and verbal communication skills in English; proficient with Microsoft Office and SharePoint.

Responsibilities

• Nurse assessments and monitoring of health status for individuals in care.
• Triage health concerns and arrange appropriate medical services.
• Document assessments and interventions per regulatory standards; maintain health records.
• Train and coach staff on health procedures, medication administration, and device use.
• Respond to nurse emergency calls on a rotating basis and participate in mandatory meetings.
