Hammer Residences, Inc. is seeking a Staff Development Nurse (RN/LPN) to advance health and wellbeing for adults with intellectual and developmental disabilities in group homes and apartments. You will train staff, coordinate medical care, and ensure regulatory compliance across programs.

The role involves nursing assessments, education, and collaboration with interdisciplinary teams. Primary duties include emergency response, health surveillance, and ongoing competency coaching for staff.

Qualifications

• Licensed as an LPN or RN in good standing with MN.
• Experience managing health services for people with intellectual disabilities.
• Valid driver’s license and driving record acceptable to Hammer’s insurer.
• Fluency in English; proficient with Microsoft Office (Outlook, Excel) and SharePoint.
• Strong organizational skills and ability to meet deadlines.

Responsibilities

• Perform nursing assessments and monitor health status for individuals in care.
• Coordinate medical services and update Support Plans with recommendations.
• Educate staff and participate in health-related training and orientation.
• Respond to nurse emergencies and support staff across sites as needed.
• Maintain documentation per regulatory and organizational standards.
• Promote HIPAA/PHI compliance and health safety procedures.
• Provide backup nursing support when assigned nurse is unavailable.
