Qualifications

Responsibilities

• Evaluate patients' clinical status and recommend rehab services per protocol.
• Provide care across all phases of cardiac rehab.
• Educate patients and families about cardiopulmonary diseases and treatments.
• Participate in case management and discharge planning with the team.
• Document patient progress and collaborate with healthcare staff.
• Develop rehabilitative goals to prevent disease progression.
• Assist in coordinating patient care and developing new programs.
