The ideal candidate will have knowledge and experience working with culturally diverse and/or vulnerable populations, including sexual minorities and persons who have mental health or substance misuse disorders. Broad knowledge and at least 1 year of experience managing HIV, STI/STD, and other communicable diseases in the clinical setting is preferred.The Specialty Care Clinic is the primary provider of Ryan White services for patients across the Northeast Health District’s 10 counties: Barrow, Clarke, Elbert, Greene, Jackson, Madison, Morgan, Oconee, Oglethorpe, and Walton.The position is based in Athens, GA and does not offer relocation assistance. If relocating to Athens, GA, please specify on the application.Job ResponsibilitiesUnder general supervision, this expanded role Public Health Nurse (PHN) works on a multidisciplinary team and is responsible for coordinating and providing assessment, planning, implementation, and related clinical services in a primary care outpatient setting for clients who have HIV/AIDS and other associated health conditions. This position plays an essential role supporting clients through the HIV Care Continuum, utilizing outreach and harm reduction principles and practices to engage and retain clients in care to achieve long-term viral suppression.General duties include but are not limited to:· Fulfill nurse protocol training requirements in HIV, STD, Immunization, and other relevant areas to function as an expanded role PHN.· Provide direct nursing care including physical assessments, education, treatment, and develop and document a plan of care consistent with the clients’ level of need.· Screen and refer clients to other community services providers as needed.
