The position focuses on developing and evaluating individualized care plans and coordinating with families and physicians.

Responsabilités

• Evaluate and update resident health care plans to provide person-centered care.
• Collaborate with healthcare team and families; communicate plans with physicians and residents.
• Document in the medical record and ensure completeness of records; follow physician orders and treatment plans.
• Provide emotional support to families and patients; deliver patient education on care plans.
• Ensure punctuality and regular attendance for assigned shifts; participate in clinical meetings.
