Keizer Court Post Acute, part of Cascadia Healthcare, seeks a dedicated Resident Care Manager (RN) to oversee patient care plans, coordinate with physicians and families, and lead the nursing team in a busy skilled nursing environment in Keizer, Oregon.

The role emphasizes person-centered care, documentation accuracy, and collaboration with interdisciplinary staff to ensure high-quality outcomes and compassionate, compliant care for residents.

Qualifications

• RN degree from an accredited nursing school.
• Current CPR certificate.
• Six months experience in a long-term care environment preferred.
• Minimum one year supervisory experience in a hospital, nursing care facility, or related health care facility.

Responsibilities

• Evaluates updates to resident health care plans to support person-centered care.
• Conducts assessments for new residents.
• Actively participates in clinical meetings.
• Collaborates with the health care team as needed.
• Uses the electronic medical record to establish and validate care parameters.
• Establishes, monitors, and documents person-centered care as indicated.
• Communicates with families and physicians about care plans and changes.
