Responsibilities

• Interview and assess patients and family, caregivers, or legal representatives.
• Determine and arrange for necessary internal and external services.
• Participate in case reviews with healthcare team to monitor treatment compliance.
• Assist with discharge planning and identify resources for transportation, housing, and social services.
• Perform Medicaid screenings as indicated.
• Assist patients with Safety Net applications when needed.
