Job summary

Mercy Health Rehabilitation Hospital in Youngstown, OH is part of Lifepoint Health. We are seeking a Case Manager, RN or Social Worker to join the team on a PRN basis to support patient care, discharge planning and collaborative care delivery.

You will participate in IPoC workflows, coordinate care conferences, and assist with utilization reviews while ensuring CMS guidelines are followed. The role emphasizes strong organization and communication in a fast-paced rehabilitation setting.

Qualifications

• Bachelor's degree in social work or RN licensure.
• Minimum of 2 years social work or case management experience in an inpatient setting.
• Current Registered Nurse or Social Work licensure or healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred
• Strong organizational and communication skills

Responsibilities

• Completes departmental orientation, initial and annual competencies.
• Assists with department performance improvement initiatives and data reporting.
• Collaborates with other staff to coordinate patient care and discharge planning.
• Completes IPoC per CMS guidelines.
• Schedules family conferences and communicates with caregivers about progress and discharge plans.
• Coordinates weekly care team conferences to refine treatment plans.
• Reviews CMG and reports missed comorbid conditions to HIM team.
• Participates in national CM Conference calls when needed.
• Assists with utilization review and appeals with payer medical directors.
• Performs intake assessment within 24 to 72 hours of admission.
• Performs follow-up assessments per Case Management Plan.
