Job summary

Lifepoint Rehabilitation, part of Lifepoint Health, seeks a Case Manager at Blanchard Valley Hospital to join our team. You will interview and assess patients and families, coordinate services, and support discharge planning to improve patient outcomes.

Applicants should hold an OH RN license, CPR certification, and have at least two years of case management experience, with strong critical thinking and the ability to work under pressure in a dynamic healthcare setting.

Qualifications

• Current RN license in OH.
• Current CPR certification.
• 2 years of relevant case management experience.
• Critical thinking and decisive judgment, with ability to work with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.

Responsibilities

• Interview and assess patients, families, caregivers, and legal representatives.
• Determine, prioritize, provide and/or arrange for needed internal and external services/interventions.
• Participate in case reviews to evaluate case management and progress; consult with healthcare team to ensure compliance with treatment plan.
• Assist with discharge planning and identify resources, including transportation, housing, healthcare, and social/spiritual services, as part of the discharge plan.
• Perform Medicaid screenings as indicated for NHP.
• Assist patients with Safety Net applications when needed.

How You'll Contribute

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.

What We're Looking For

Applicants should have a current RN license in the state of OH. Additional requirements include:

• Current CPR certification


• 2 years of relevant experience required
