Job summary

Strive Recovery in Fort Wayne, IN is seeking a dedicated RN or LPN to provide compassionate resident care on 12-hour shifts, with rotating weekends. You will assess needs, develop and follow individualized care plans, administer medications, and coordinate admissions or discharges.

Qualifications

• Current Indiana RN or LPN license.
• Current CPR certification.
• Experience in behavioral health or geriatrics is a plus.

Responsibilities

Job Description

• Be responsible for the care and well-being of the residents in your charge

• Assess the residents’ needs and assist in developing and following individual care plans

• Assure quality care is delivered through the utilization of nursing processes and guidelines

• Identify and communicate changes in residents’ status to physicians, staff, and family members

• Administer medications, treatments, and other direct care as prescribed and indicated

• Coordinate admissions, transfers, and discharges

• Communicate appropriate information to all staff to provide a smooth shift transition

• Investigate and document incidents and accidents as required

• Document appropriately as needed or required in PCC
