Job summary

Strive Recovery in Fort Wayne, IN is seeking dedicated nursing professionals to provide compassionate, evidence-based behavioral health care for residents. Shifts include 1st and 2nd with rotating weekends, and duties cover medications, care plans, and care coordination.

Applicants must hold an Indiana RN or LPN license and CPR certification. The role emphasizes quality nursing processes, team communication, and a supportive growth environment.

Responsibilities

• Be responsible for the care and well-being of the residents in your charge.
• Assess the residents’ needs and assist in developing and following individual care plans.
• Assure quality care is delivered through the utilization of nursing processes and guidelines.
• Identify and communicate changes in residents’ status to physicians, staff, and family members.
• Administer medications, treatments, and other direct care as prescribed and indicated.
• Coordinate admissions, transfers, and discharges.
• Communicate appropriate information to all staff to provide a smooth shift transition.
• Investigate and document incidents and accidents as required.
• Document appropriately as needed or required in PCC.
