Job summary

Strive Recovery in Fort Wayne, IN is seeking a dedicated RN or LPN to provide compassionate resident care on 12-hour shifts, with rotating weekends. You will assess needs, develop and follow individualized care plans, administer medications, and coordinate admissions or discharges.

Qualifications

Current Indiana RN or LPN license.
Current CPR certification.
Experience in behavioral health or geriatrics is a plus.

Responsibilities

Care for residents and ensure well-being under nursing direction.
Assess residents’ needs and support development of care plans.
Ensure quality care through nursing processes and guidelines.
Identify and communicate changes in residents’ status to physicians and family.
Administer medications, treatments, and direct care as prescribed.
Coordinate admissions, transfers and discharges; document appropriately.
