Job summary

Hospice of the Midwest is seeking an On-Call Registered Nurse to join our team in Johnston, IA. The role involves after-hours, hands-on hospice care, collaborating with the care team to support patients and families in a compassionate manner.

The RN must hold an active nursing license in the state and have at least one year of nursing experience, with hospice exposure preferred. A valid driver's license is required, and the role offers stability, advancement opportunities, and comprehensive

• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting is preferred.

Responsibilities

• Provide 1:1 care to hospice patients during on-call hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols or orders.
• Communicate with physicians regarding changes in condition and obtain orders as necessary.
• Offer guidance to patients and families about current conditions and next steps.
• Pronounce death when applicable during on-call hours and assist the family with next steps.

Schedule: Full Time Monday-Thursday 5PM-8AM

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• Assess and manage urgent symptoms quickly and compassionately

• Administer medication or adjust care based on hospice protocols or provider orders

• Communicate with the physician regarding changes in condition and obtain orders as necessary

• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps

• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team

• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation

• One year nursing experience required, 3+ years preferred

• Experience in hospice or a similar setting preferred

• Knowledge of the hospice philosophy of care

• Commitment to clinical and documentation excellence

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
