Job summary

Hospice of the Midwest is seeking a dedicated On-Call Registered Nurse to join our team in Sioux City, IA. The role involves providing 1:1 hospice care during after-hours, supporting patients and families with compassion and clinical excellence.

You will work on a weekend on-call schedule (Friday 5PM–Monday 8AM) and collaborate with the Remote Triage On Call Team to deliver hands-on care and guidance as needs arise.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an insured automobile.

Responsibilities

• Provide 1:1 care to patients and families during after-hours on-call shifts.
• Collaborate with Remote Triage On Call Team to deliver hands-on hospice care.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols or orders.
• Communicate with the physician regarding changes in condition and obtain orders.
• Offer guidance to patients and families on current condition and next steps.
• If death occurs during on-call hours, pronounce death and provide family support, contact funeral home, and notify hospice team.
• Complete timely documentation and communication.

As a On-Call RN You will:

• Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• Assess and manage urgent symptoms quickly and compassionately

• Administer medication or adjust care based on hospice protocols or provider orders

• Communicate with the physician regarding changes in condition and obtain orders as necessary

• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps

• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team

• Complete timely documentation and communication
