Job summary

Hospice of the Midwest is seeking a compassionate On-Call Registered Nurse for Edina and Monticello, MN. This part-time role requires after-hours coverage on weekends and holidays, providing 1:1 patient care and family support with a focus on symptom management and comfort.

The RN will collaborate with the care team, administer medications per orders, and document thoroughly while ensuring high-quality end-of-life care. A driver's license, CPR, and hospice experience are preferred.

Qualifications

• Graduate of an accredited nursing school with current licensure in the practicing state.
• One year nursing experience required, 3+ years preferred.
• Must be a licensed driver with an insured vehicle.
• Maintain current CPR certification where required.

Responsibilities

• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care per hospice protocols or orders.
• Communicate with physician regarding condition changes and orders.
• Provide guidance and support to patients and families about current condition and next steps.
• Complete timely documentation and communications.

Schedule: On Call Every Other Sat & Sun, 8:00 AM - 8:00 PM

As a On-Call RN You Will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
