Job summary

Hospice of the Midwest is seeking a dedicated Hospice RN to join our Monticello, MN team. The role focuses on delivering patient-centered, compassionate end-of-life care in a home setting. The RN will work closely with a multidisciplinary team to create personalized care plans and ensure quality, compliant nursing care.

Qualifications

• Graduate of an accredited nursing program with RN licensure in the operating state.
• Minimum of 1 year nursing experience; 3+ years preferred.
• Hospice experience or related setting preferred.
• Knowledge of hospice philosophy of care.
• Valid driver's license and insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create and implement individualized hospice care plans.
• Direct nursing care including medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team; maintain accurate documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
