Hospice of North Alabama is seeking a dedicated RN for Hospice Admissions in Decatur, AL. You will assess patients, explain services, and coordinate start of care across nursing, aides, and equipment, working within Morgan and Lawrence counties. Office location is Cullman, with Monday–Friday, 8am–5pm scheduling.

Experience in hospice preferred; strong clinical assessment, documentation, and teamwork are essential to support patients and families during a critical time.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in Alabama.
• Minimum of 1 year hospice nursing experience; 3+ years in LTC/ICU/ER considered.
• Must be a licensed driver with insured vehicle and valid registration.
• Maintain current CPR certification where required.

Responsabilités

• Visit patients in homes, hospitals, nursing facilities, etc. referred for hospice care.
• Explain hospice services and discuss goals with patient and families; obtain consents and documents.
• Obtain medical history, current meds, and symptoms; perform clinical assessments.
• Determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate initiation of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team and maintain timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
