Job summary

Hospice of North Alabama is seeking a compassionate Hospice Admissions RN to join our team serving the Cullman and Decatur areas in Alabama. You will conduct patient visits in homes, hospitals, and facilities, assess eligibility for hospice, and coordinate service initiation with the care team.

The role emphasizes independent work with team support, adherence to Medicare guidelines, and delivery of high-quality, patient-centered care with a focus on comfort and dignity for patients and families.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, ICU, or Emergency room will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
• Possess and maintain CPR certification in states where required.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care.
• Explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
