BrightSpring Health Services is seeking a Hospice RN Case Manager to join Generations Hospice. The role focuses on compassionate, patient-centered hospice care with strong interdisciplinary team support and after-hours triage.

Office location is Fond du Lac, WI, with travel across Winnebago & Fond du Lac Counties. Schedule is Monday–Friday 8 AM–5 PM, offering a $7,500 sign-on bonus.

Qualificações

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile insured as required

Responsabilidades

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient’s wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Hospice RN Case Manager - Where Support Meets Impact

• Lower patient-to-nurse ratios for meaningful, unrushed hospice care
• Strong interdisciplinary team support-collaborative care with Social Work, Chaplaincy, Aides, and providers, so you\'re never practicing alone
• Leadership that understands hospice nursing
• Dedicated internal after-hours triage team to support nurses outside of regular business hours
• Homecare Homebase (HCHB) EMR for streamlined documentation
