Job summary

Generations Hospice is seeking a Registered Nurse Case Manager to provide patient-centered hospice care across Wisconsin. The role includes direct nursing care, care planning, and collaboration with an interdisciplinary team while supporting families and ensuring thorough documentation.

Travel throughout the territory is required, with office in Fond du Lac. A strong nursing background in hospice and a current RN license are essential for delivering compassionate, high-quality care to patients

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Hospice RN Case Manager - Where Support Meets Impact

Coverage area: Travel throughout the territory is required - Serving Winnebago & Fond du Lac Counties

Schedule: Monday-Friday 8AM-5PM
