Job summary

Generations Hospice, an affiliate of Adoration Health, is seeking a compassionate RN Case Manager to deliver patient-centered end-of-life care in a home-based setting. You will assess needs, develop care plans, administer treatments, and educate families.

We offer a $5,000 sign-on bonus, strong interdisciplinary support, flexible scheduling, and opportunities for growth. Travel throughout the territory is required to serve Winnebago & Fond du Lac Counties.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the operating state.
• Minimum 1 year nursing experience; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care and clinical documentation excellence.
• Must have a valid driver’s license and reliable insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient goals.
• Direct nursing care including medications, treatments, and interventions.
• Provide pain and symptom management and family education.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by the local branch.

Responsibilities

As a RN Case Manager You will:

• Assess/monitor physical, emotional, and psychological needs of patients

• Create hospice care plans that align with the patient's wishes and goals

• Direct nursing care: administering medications, treatments, and interventions

• Provide pain and symptom management

• Educate and support the patient’s family and caregivers

• Collaborate with interdisciplinary team

• Maintain accurate and timely documentation

• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
