Job summary

Generations Hospice is seeking a dedicated Hospice RN Case Manager to provide expert, patient-centered care at a critical time in patients' lives. You will work with an interdisciplinary team to create care plans, administer medications, and support families in a compassionate setting.

Office is in Fond du Lac, WI with a Monday–Friday schedule. $7,500 sign-on bonus for external/new hires. Join a growing team that values clinical excellence and work-life balance.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Coverage area: South Fond du Lac areas & Beaverdam/Mayville areas

Schedule: Monday-Friday 8AM-5PM
