Job summary

Generations Hospice is seeking a compassionate RN Case Manager to provide expert hospice nursing care in Wisconsin. You will assess patient needs, develop care plans, administer medications and treatments, and educate families, with strong team collaboration and documentation.

This role offers a $7,500 sign-on bonus and a Monday–Friday schedule in Fond du Lac/Horicon area, with supportive leadership and a focus on work-life balance.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured automobile in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
