Job summary

Generations Hospice is seeking an experienced RN Case Manager to provide patient-centered hospice care in a home-based setting. You will assess needs, develop care plans, administer treatments, and collaborate with an interdisciplinary team to ensure compassionate, comprehensive care for patients and families.

The role emphasizes clinical excellence, thorough documentation, and on-call availability within the Fond du Lac/Winnebago county area, with a Monday–Friday schedule and generous benefits.

Responsibilities

• Assess/monitor patients' physical, emotional, and psychological needs.
• Create hospice care plans aligned with patient goals.

• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team and maintain documentation.
• Participate in on-call rotation as required by local branch.

Responsibilities

As a RN Case Manager You will:

• Assess/monitor physical, emotional, and psychological needs of patients

• Create hospice care plans that align with the patient's wishes and goals

• Direct nursing care: administering medications, treatments, and interventions

• Provide pain and symptom management

• Educate and support the patient’s family and caregivers

• Collaborate with interdisciplinary team

• Maintain accurate and timely documentation

• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
