Qualifications

• Lower patient-to-nurse ratios for meaningful, unrushed hospice care

Office Location: 1028 S. Main Street Suite C, Fond du Lac, WI 54935. Coverage area: Serving Winnebago & Fond du Lac Counties; travel throughout the territory is required. Schedule: Monday‑Friday 8AM‑5PM.

As a RN Case Manager You Will:

• Assess/monitor physical, emotional, and psychological needs of patients
