Job summary

Generations Hospice is seeking a dedicated Hospice RN Case Manager to provide patient-centered, 1:1 hospice care with strong interdisciplinary support. This role emphasizes collaborative care with social work, chaplaincy, aides, and providers, ensuring meaningful outcomes for patients in the WI-OMRO area.

Enjoy a reduced patient-to-nurse ratio, dedicated after-hours triage, and a supportive leadership team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with a dependable vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
