Job summary

Hospice of North Alabama is seeking a dedicated Registered Nurse to join our team in Decatur, AL. The RN Case Manager will provide patient-centered hospice care in home settings and collaborate with an interdisciplinary team to develop and implement individualized care plans.

Requirements include an active RN license in Alabama, at least 1 year of nursing experience (3+ years preferred), and CPR certification. We offer comprehensive benefits and a supportive work environment.

Qualifications

• RN licensure required in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer meds, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• - Graduate of an accredited nursing school with current licensure to practice in state of operation
• - One year nursing experience required, 3+ years preferred
• - Experience in hospice or a similar setting preferred
• - Knowledge of the hospice philosophy of care
• - Commitment to clinical and documentation excellence
• - Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• - Possess and maintain current CPR certification in states where required.
