The role you’ll contribute:
The RN Care Manager collaborates with patients, families, social workers, nurses, physicians, and the interdisciplinary team to ensure patient-centered care coordination and progression through the continuum of care. Responsibilities include developing transition of care plans, monitoring appropriate resources, and escalating clinical care as needed. Under supervision of the Care Management Supervisor, Manager, or Director of Nursing, the RN Care Manager evaluates post-hospital needs, facilitates safe and efficient patient transitions, and manages patient flow to improve continuity of care, patient satisfaction, safety, and readmission prevention.
The RN Care Manager participates in daily multidisciplinary rounds, manages discharge planning, and understands medical necessity. They facilitate collaboration across the continuum, remove barriers to care, and educate the team on resource utilization, medical necessity, CMS CoP, and care coordination. Knowledge of post-hospital services such as home health, infusion, durable medical equipment, palliative and hospice care, outpatient services, transition clinics, skilled nursing, rehabilitation, and community organizations is essential. The role adheres to policies, ensures quality and compliance, and promotes outstanding customer service and respectful relationships.
