The Hospital Care Coordinator-RN Case Management responsibilities include:
  1. Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
  2. Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
  3. Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
  4. Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
  5. Ability to adjust to changing priorities in a fast-paced environment.
Position details:
  1. Location: Morton Plant North Bay Hospital - New Port Richey, FL
  2. Status: PRN, Work as Needed
  3. Schedule: Days will vary; 8:00 AM - 4:30 PM
  4. Weekend Requirement: Every 3rd
  5. On Call: No
Certifications and Licensures:
  1. Required: RN (Registered Nurse)
  2. Preferred: ACM (Case Management)
  3. Preferred: CCM (Case Manager)
Education:
  1. Required: Associates of Nursing
  2. Preferred: Bachelor of Nursing
Experience:
  1. Required: 2 years in Acute Care
  2. Required: 2 years Registered Nurse
