The RN Care Manager collaborates with patients, families, social workers, nurses, physicians, and the interdisciplinary team to ensure patient-centered care coordination and progression through the continuum of care. Responsibilities include ensuring efficient and cost-effective care through resource monitoring and escalation of clinical care when necessary. The RN Care Manager evaluates post-hospital needs, develops transition of care plans, and initiates their implementation prior to patient discharge. The role aims to optimize patient flow, ensure safe transitions, enhance patient satisfaction, safety, and prevent readmissions.
Daily communication with the interdisciplinary team during rounds is essential. Core competencies include care coordination, discharge planning, transition of care, and understanding medical necessity. The RN facilitates collaborative management across the continuum, removing barriers to care and reimbursement. Education on resource utilization, medical necessity, CMS CoP, and post-hospital services is provided. Knowledge of available post-hospital care options such as home health, infusion, DME, palliative and hospice care, outpatient services, transitional clinics, SNFs, rehab, and community organizations is required. The RN Care Manager adheres to policies, ensures quality and compliance, and participates in delivering excellent customer service while maintaining respectful relationships.
