Job Duties

The RN Care Manager in the ED works with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Ensure the patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and evaluations. Assess and respond to patient and family needs by coordinating efforts of other team members. Identify and resolve barriers that hinder effective patient care.

A flexible RN opportunity with a 4-day work week! This position is full-time, 10-hour shifts, 40 hours weekly. Three (3) years of RN work experience is required. Union, yes.

The successful candidate will work in the Emergency Room, closely collaborating with the ED interdisciplinary team to support the Emergency Room patient population.

  • Collaborate with physicians, nurses, interdisciplinary team, and others to assess, plan, and initiate the patient plan of care.
  • Facilitate and coordinate discharge details to appropriate agencies.
  • Develop and coordinate an individualized care plan.
  • Serve as a resource regarding available agency, facility, and community services to assist in discharge planning.
  • Complete appropriate federal, state, and local forms.
  • Ensure adherence to quality standards and participate in quality monitoring and improvement by monitoring care quality, reporting risks, and entering data to support quality initiatives.
  • Conduct ongoing case reviews for delays or variances in care; raise issues with relevant departments and physicians.
  • Capture data as per department policy.
  • Communicate issues for resolution and intervention to appropriate physicians.
  • Use approved medical necessity criteria to ensure patient appropriateness of setting.
  • Involve payer liaison in cases potentially not meeting medical necessity guidelines.
  • Support the payer liaison role to resolve barriers and avoid third-party payer denials.
  • Maintain awareness of all concurrent denials through interaction with payer liaison.
  • Work with payer liaison to facilitate timely discharges or alternative care plans acceptable to all parties.

Education: Graduate from Specialty Training Program-Nursing (Required); Bachelor's Degree-Nursing (Preferred)
