Job Summary

The Clinical Care Manager (CCM) coordinates patient care collaboratively with the inter-professional care team, patients, and families to achieve quality outcomes through efficient, evidence-based, culturally sensitive care. Responsibilities include monitoring patient outcomes, applying best practice nursing strategies, coaching staff, resolving unit issues or patient complaints, leading patient flow, overseeing patient experience, and contributing to policy development. The CCM designs and implements care strategies tailored to disease-specific populations as needed.

Qualifications
Education and Credentials
  • Graduate of an accredited BSN nursing program.
  • Current NC RN license or licensure from a Nursing Compact State.
  • BCLS certification, with American Heart Association certification required within three months if not already held.
  • MSN or related master's degree required for certain positions, to be completed within five years of employment.
Additional Practice Area Requirements
  • Emergency Department: ACLS, PALS/ENPC, and Non-Violent Crisis Intervention Certification.
  • Psychiatry Services: Non-Violent Crisis Intervention Certification.
  • Birthing Center: ACLS and NRP.
  • Nurseries: NRP, PALS, and STABLE certifications.
  • Pediatrics: NRP and PALS.
  • PACU and Day Surgery: ACLS and PALS within one year of employment.
Preferred Qualifications
  • Master’s degree in Clinical Nurse Leader, certification, or specialty certifications.
Work Experience
  • Minimum of five years of nursing experience, including leadership roles, with proficiency expected within three months of employment.
  • Experience in clinical care coordination preferred.
