The RN Care Manager in the ED works with physicians and multidisciplinary team members to develop the plan of care for each assigned patient from admission through discharge. They ensure patient progress towards desired outcomes by continuously monitoring care through assessments and evaluations. The role involves assessing and responding to patient and family needs by coordinating efforts of other team members, as well as identifying and resolving barriers to effective patient care.
This is a flexible RN opportunity with a 4-day work week. The position is full-time, with 10-hour shifts totaling 40 hours weekly. A minimum of three (3) years of RN work experience is required. The position is unionized.
The successful candidate will work in the Emergency Room, collaborating closely with the ED interdisciplinary team to support the emergency patient population.
