Job Description:

Works collaboratively with physicians, interdisciplinary teams, patients, and families to promote positive patient outcomes. Performs a care continuum process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to support the individual's health needs. Utilizes skilled communication, education, and resources to promote quality, cost-effective outcomes. Provides focused support in areas such as utilization management, emergency department, acute, ambulatory, and specialty care.

Essentials

  • Demonstrates critical thinking skills when utilizing the nursing process, based on research, evidence-based outcomes, and Standards of Practice to meet patient healthcare needs.
  • Establishes collaborative partnerships with patients to examine health care needs, decisions, lifestyle choices, and resource utilization affecting their health.
  • Advocates, educates, and coaches patients, families, and caregivers about treatment options, community resources, and psychosocial concerns to set goals and develop self-care skills and independence appropriate to their age and developmental level.
  • Facilitates communication and coordination among healthcare team members, involving patients in decision-making to minimize fragmentation of services.

Qualifications

  • Graduate of an accredited Registered/Professional Nursing program.
  • Required Licensure/Certification: State licensure as a Registered Nurse (RN).

Preferred Qualifications

  • Bachelor’s degree in Nursing.
  • Certified Case Manager (CCM) or relevant certification as determined at the position level.
  • Two years' nursing experience in outpatient care.
  • One year of Care Management experience.

Physical Requirements:

Requires visual acuity to read information, labels, monitors, and assess patient needs. Must communicate effectively with patients, providers, and visitors, especially during emergencies. Manual dexterity to manipulate complex equipment and frequent computer use for documentation.

Scheduled Weekly Hours: 40

The hourly rate ranges from $37.98 to $58.61, dependent on experience.
