Job summary

Complete Care Management is seeking an RN Registered Nurse to provide direct care to residents recently discharged from the hospital or in long-term care. You will administer medications, perform wound care and monitor health status.

You will develop individualized care plans, collaborate with physicians, therapists and social workers, and educate families. Documentation and team supervision ensure high-quality care.

Qualifications

• Must be licensed to work in CT as an RN
• Able to pass background check, drug screen, references, and physical
• Demonstrates professional and clinical leadership qualities

Responsibilities

• Administer medications (oral, topical, IV, injections)
• Perform wound care and pain management; apply dressings
• Conduct health assessments and monitor changes; analyze data
• Create, update and execute individualized care plans
• Collaborate with physicians, therapists, social workers and others
• Educate residents and families about health status and care expectations
• Document health status, care provided and patient progress
• Oversee and delegate tasks to LPNs and CNAs to ensure quality care

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Treatments: Perform wound care/prevention, manage pain, apply dressings

Assessment & Monitoring: Conduct health assessments, track changes & analyze data

Care Planning: Create, update & execute individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate to LPN’s and CNA’s ensuring quality care/teamwork
