MEDI KO in Olympia, WA is hiring a Reentry Targeted Case Manager (RN licensure required) to provide recovery-focused care coordination for Apple Health clients during pre-release. This role centers on assessment, care planning, and linkage to community resources within a correctional healthcare setting.

You will collaborate with clinical teams and Medicaid partners to ensure compliant documentation, billing, and seamless continuity of care as clients transition back to the community.

Formación

• Active licensure allowing assessment and care planning across medical and behavioral health needs (e.g., RN).
• Active CPR/BLS certification required where role is in a correctional facility.
• Relevant industry certifications may be required (PHR, SHRM-CP, CPA, etc.).
• Experience in healthcare, correctional healthcare, or a regulated industry is preferred.

Responsabilidades

• Provide person-centered, recovery-focused care coordination during the pre-release period.
• Complete reentry assessments and develop comprehensive care plans.
• Coordinate medical and behavioral health services and connect clients to community resources.
• Ensure documentation supports Medicaid billing and state regulations.
• Collaborate with clinical teams, BH providers, community partners, and MCOs to ensure continuity of care and reduce gaps in service.
