MEDIKO is seeking a Reentry Targeted Case Manager (RN licensure required) at the Nisqually Correctional Facility in Olympia, WA. Qualifikationen

• Active licensure that permits assessment and care-planning across medical and behavioral health needs (e.g., RN).
• Active CPR/BLS certificate required (AHA/ARC guidelines), if role is in a correctional facility.
• Knowledge of regulations, compliance standards, and best practices may be required.
• Minimum 2–3 years of relevant experience in the functional area.
• Must meet provider enrollment requirements under applicable state regulations.

Aufgaben

• Provide person-centered, recovery-focused care coordination to justice-involved Apple Health clients.
• Complete reentry assessments and develop care plans.
• Coordinate medical and behavioral health services and community resources.
• Ensure Medicaid billing documentation and facility standards are met.
• Collaborate with clinical teams, behavioral health, and external partners to ensure continuity of care.
