Job summary

Washington State Penitentiary in Walla Walla, WA seeks an Internal RN2 for Discharge Planning. The role leads Continuity of Care planning, coordinating with health services, community resources, and DOC staff to ensure safe release and follow-up care for high‑risk patients.

Responsibilities include coordinating referrals, maintaining records, and collaborating with nurses, counselors, and security to support successful reentry and reduce recidivism. Apply early as recruitment is ongoing.

Qualifications

• Licensed Registered Nurse in Washington state.
• BLS certification required; ACLS not a substitute.
• Proficient with computer-based systems and data entry.
• Ability to work independently in a multidisciplinary team.

Responsibilities

• Initiate and support Continuity of Care planning for discharged patients.
• Coordinate with community partners and DOC Health Services staff.
• Maintain complete, confidential patient records per policies.
• Facilitate communication among health, security, and administrative teams.
