Job summary

Paradigm is seeking a Minnesota QRC Nurse Case Manager to provide rehabilitation and case management services across the Minneapolis/St. Paul region.

You will consult, develop, and implement rehabilitation plans, working independently with consumers, providers, and carriers to help injured workers return to work. Qualified candidates will have MN RN licensure, QRC internship or registration, and relevant clinical experience.

Qualifications

• Active MN RN license.
• QRC registration preferred; internship required if not yet registered.
• Two years of direct clinical experience with consumers.
• Current, unrestricted license to practice in MN or other states.

Responsibilities

• Accepts cases for referral and management per Minnesota Rules.
• Counsel and motivate consumers toward rehabilitation goals.
• Maintain communications with consumers, families, providers, and employers.
• Prepare accurate, timely reports per Minnesota rules.
• Ensure rehabilitation plan is implemented and progress documented.
• Support sales/marketing to build caseload and client relationships.
