Job summary

Paradigm is seeking a full‑time Nurse Case Manager to cover the St. Cloud area. The role involves consultation, development, and implementation of rehabilitation plans for injured workers, working independently to support recovery and return‑to‑work goals.

You will interact with injured workers, their families, employers, physicians, and insurers in a collaborative, compliant setting. The position requires MN RN licensure, potential QRC certification, and relevant case management experience; a

Qualifications

• Current, unencumbered RN license in MN.
• Minnesota Qualified Rehabilitation Consultant (QRC) preferred; internship available if not yet registered.
• Two years of direct clinical experience with consumers (required for cases from Accounts).
• Licenses/certifications must be current and allow practice within the US.
• Valid driver’s license and clean driving history.

Responsibilities

• Accept cases for referral and management per Minnesota rules.
• Counsel, direct and motivate consumers to reach maximum function.
• Establish relationships and maintain communications with consumers, families, providers, and employers; maintain confidentiality.
• Complete accurate written reports per Minnesota rules and documentation standards.
• Ensure rehabilitation plan is implemented and progress is documented.
• Assist sales/marketing to build caseload and develop client relationships.
