Paradigm is seeking a Minnesota-qualified Rehabilitation Consultant (QRC) Nurse Case Manager to provide rehabilitation and case management services to injured workers in the Saint Cloud area. The role requires performing consultation, development, and implementation of rehabilitation plans while working independently with various stakeholders to support return-to-work goals.

Qualifications include an MN RN license with QRC preferred; applicants may pursue Minnesota QRC internship if not yet

Qualifications

• Current, unencumbered RN license in MN.
• QRC registration preferred; internship available for non-registered candidates.
• Knowledge of Minnesota Workers' Compensation statutes and vocational rehabilitation regulations.
• Regional or national certifications may be required by certifying bodies.
• Two years of direct clinical experience with consumers.
• Licenses must be current and allow practice in the United States.
• Valid driver’s license and good driving history.

Responsibilities

• Accept cases for referral and management per Minnesota rules.
• Counsel, direct and motivate consumers to reach their highest level of function.
• Establish and maintain communications with consumers, families, providers, carriers, and employers with confidentiality.
• Complete accurate and timely written reports per Minnesota rules.

Job description

Paradigm is seeking a Minnesota Qualified Rehabilitation Consultant (QRC) Nurse Case Manager to provide rehabilitation and case management services to injured workers throughout the Saint Cloud area.

This position is responsible for the consultation, development, and implementation of the rehabilitation plan. A QRC works independently using their own discretion to assist injured workers/consumers in their recovery so that they may return to work. The QRC works with all parties to the claim including: the injured person, the claim’s examiner, employer, attorneys (plaintiff and defense) and medical providers.

Responsibilities:

• Accepts cases for referral and management in accordance with Minnesota Rules.
• Counsel, direct and motivate consumers to reach their highest level offunction.
• Establishes relationships and maintains communications with consumers, families, providers, Paradigm’s carrier clients and employers. Ensures that all communication is culturally sensitive and meets requirements to maintain injured worker confidentiality.
• Completes accurate and timely written reports in accordance with Minnesota rule and documentation standards.
• Ensures that rehabilitation plan is implemented and documents progress towards achievement of the rehabilitation plan.
• Assist sales/marketing team as needed in order to build caseload. Develop solidrelationships with customers in order to assure repeatbusiness.

Qualifications:

• Current, unencumbered Registered Nurse (RN) license in MN
• Registered Minnesota Qualified Rehabilitation Consultant (QRC) preferred. Candidates who are not yet registered must complete a Minnesota QRC internship, provided by Paradigm.
• Knowledge of Minnesota Workers' Compensation statutes and vocational rehabilitation regulations.
• Case Managers may also need regional or national certifications and must adhere to the standards of practice outlined by the specific national certifying body or the jurisdictionally accepted standards of practice.
• Two years of full-time equivalent direct clinical experience to the consumer (required forcasework referred from Accounts requiring accredited case management).
• Professional licenses or certifications required to meet qualifications for this position mustbe current, unrestricted and allow for practice within a state or territory of the UnitedStates.
• Valid driver’s license, and good driving history

Competencies:

• Exceptional time management and organization skills are required. A full-time QRCis responsible for approximately 25 files that all require contact and reporting within specific timelines.
• The ability to communicate effectively with a wide range of people on a daily basis utilizingboth verbal and writtencommunication.
• Flexibility, versatility, and problem solving – this role works at a fastpace.
• Technology proficiency – QRC’s at Paradigm CCS use an advanced CaseManagement software solution to do their work. A laptop and cell phone are useddaily.
