Job summary

Paradigm is seeking a full-time Nurse Case Manager for the Minneapolis/St. Paul area. This role involves consultation and implementation of rehabilitation plans for injured workers. The Nurse Case Manager will work independently while collaborating with various stakeholders. Candidates must have a current Registered Nurse license and preferably a Minnesota Qualified Rehabilitation Consultant credential. The position offers autonomy, a supportive culture, and the opportunity to influence recovery outcomes.

Qualifications

• Current Registered Nurse license in Minnesota.
• Preferred registration as Minnesota Qualified Rehabilitation Consultant.
• Two years of clinical experience in case management.

Responsibilities

• Accept cases for referral and manage according to Minnesota Rules.
• Counsel and motivate consumers toward recovery.
• Establish relationships and maintain communications with healthcare teams.
• Complete timely reports as per documentation standards.
• Implement rehabilitation plans and document progress.
• Support sales/marketing efforts as needed.
