• Care Planning: Develop, update, and manage individualized resident care plans with physicians and the interdisciplinary team.
• Documentation: Maintain precise medical records and ensure regulatory compliance.
• Communication: Serve as the primary clinical liaison for physicians, family members, and facility administration.
