Job summary

White Blossom Care Center in San Jose, CA is seeking a Registered Nurse (RN) Supervisor for day and evening shifts on a 4 on/2 off rotation. The role provides clinical leadership to LVNs and CNAs, ensures regulatory compliance, and supports daily operations with a focus on resident care and team collaboration.

The supervisor will oversee admissions, maintain documentation, and partner with physicians to update care plans, while guiding staff to deliver high-quality care in a resident-centered

Qualifications

• Active California RN license required.
• Current CPR/BLS certification.
• Prior charge nurse or supervisory experience in SNF or LTC preferred.
• Experience with PointClickCare (PCC) for EHRs and audits is a plus.
• Familiar with Title 22 long-term care regulations.

Responsibilities

• Oversee admissions and clinical documentation on assigned shifts.
• Supervise and mentor floor nurses and CNAs with performance feedback.
• Conduct digital clinical audits in PCC to ensure medical record accuracy.
• Maintain shift-wide regulatory compliance with Title 22 and policies.
• Collaborate with physicians and the interprofessional team to update care plans.
