Job summary

Loyola Medicine, a member of Trinity Health, seeks a Lung Transplant Evaluation Coordinator to manage the evaluation process for potential transplant candidates and serve as the main contact for patients, caregivers, and providers.

The role emphasizes coordinating testing, education, and regulatory documentation while ensuring compliance with transplant program standards and delivering patient-centered care. Experience in transplant settings is highly valued.

Qualifications

• Three years of professional nurse experience; at least 1 year transplant, outpatient or dialysis experience desirable.
• ADN required; BSN preferred.
• Current IL RN license CPR (Cardiopulmonary Resuscitation).
• National Certification as Transplant Coordinator or related specialty preferred.

Responsibilities

• Coordinate care using established clinical protocols.
• Review tests, labs, and data to support decision-making.
• Document in progress notes, flow sheets, Phoenix database, and the EHR.
• Collaborate with physicians, social workers, financial coordinators, and nursing staff.
• Manage triage calls and partner with physicians to address patient care needs.
• Participate in program and committee meetings.
• Submit information to insurance carriers for precertification/authorization.
• Provide cross-coverage to fellow coordinators as needed.
• Create and maintain educational resources for patients, families, staff nurses, and program teams.
• Collect, report, and submit data to the Pulmonary Hypertension Association per accreditation standards.
• Prepare quality metrics, attend protocol meetings, and support QA/PI activities.
• Engage in community outreach and travel to partner facilities as needed.
• Serve as a pulmonary hypertension content expert locally and nationally.
