Qualifications

• Must be a graduate of an Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing.
• Licensed as a Registered Nurse by the Indiana Professional Licensing Agency required.
• 3+ years nursing experience, preferably in Population Health management; home health, ambulatory care, community public health, case management, coordinating care across multiple settings and providers.
• Experience with electronic health records, EPIC experience preferred.
• Knowledge and experience teaching self-management preferred.
• Experience working with a Population Health Team &/or Accountable Care Organization preferred.

Responsibilities

• Provide patient-centered care coordination across multiple settings and providers.
• Engage in telephonic and in-person interactions to support patients and care teams.
• Contribute to population health initiatives and care management programs.
