Trillium Health Resources is seeking a Population Health Nurse to provide clinical care management and population health support for assigned members, focusing on prevention, wellness, and outcomes. Qualifications

• Associate’s degree in Nursing and one year of RN experience.
• Active North Carolina RN licensure.
• Must have a valid driver’s license.
• Must reside within North Carolina.

Responsabilités

• Conduct clinical assessments and identify member health needs and care gaps.
• Develop and implement care plans for preventive care and chronic condition management.
• Coordinate services across providers and community resources.
• Monitor progress and adjust care plans based on outcomes.
• Provide health education and coaching to support self-management.
