Job summary

JMH is seeking a registered nurse to provide patient service coordination across the surgical service line, navigating pre-operative, hospital admission and post-acute care. The role collaborates with patients, families, physicians, and payers to optimize outcomes and cost efficiency.

The candidate should have a BSN (MSN preferred), current RN license, and experience in discharge planning, utilization review, or case management.

Qualifications

• Bachelor of Science in Nursing required; Master’s preferred.
• RN required in-state licensure where applicable; California Board of Nursing referenced in source.
• BLS certification required.
• CCM or ACM certification preferred.
• Experience with perioperative care, discharge planning, utilization review, or case management recommended.

Responsibilities

• Coordinate pre-operative, admission, and post-acute phases of care.
• Align perioperative services with patient, family, and providers; ensure ERAS compliance where applicable.
• Identify opportunities to improve patient experience, reduce length of stay, and manage costs.
