Qualifications

• RN license in California is required.
• BLS certification required.
• CCM or ACM certification preferred.
• Experience in discharge planning, utilization review or case management is required.
• Experience with elective orthopedic surgical patients is preferred.

Responsibilities

• Coordinate patient services for the perioperative pathway across pre-op, admission, and post-acute phases.
• Collaborate with patients, families, physicians, clinical team and payers as needed.
• Ensure compliance with ERAS and related payment model requirements for each surgery.
• Identify opportunities to improve patient experience, reduce length of stay and optimize costs.
